Job Title: Latino Health Navigator & Clinic Coordinator

Street Level Health Project is looking for a committed and passionate person to work with
us in immigrant health.  This position will be responsible for two programs: Latino Health
Navigation and our twice weekly health clinic.  The Latino Health Navigator works to
support individuals as they advocate for themselves and their right to healthcare.  He or she
will maintain a strong collaborative relationship with our community partners to ensure
quality healthcare for our clients.  The Clinic Coordinator oversees the daily operations of
our drop-in triage clinic and works to ensure the sustainability of the clinic, including our
volunteer staff.  This person will report directly to the Executive Director.  The position is
32 hours, and split between two roles, as needed.

SLHP is dedicated to improving the health and well-being of underserved, urban, immigrant
communities in the Bay Area.  We are an entry point to the health care system for the
uninsured, underinsured and recently arrived.  At SLHP, we develop trusting relationships
with new immigrants, offer them a place to build a healthy and vibrant community, and
empower them to advocate for the well-being of themselves and their families.

Responsibilities as Latino Health Navigator
  • Connect clients with necessary social services, such as food, housing, mental health
    and substance abuse treatment.
  • Provide supported referrals to specialty and chronic care, as directed by the physician.
  • Provide resources and information for clients in appropriate language(s).
  • Develop our referral network with other organizations, including maintenance of
    referral system, and follow-up on referrals.
  • Maintain medical record database.
  • Work with Executive Director to provide technical assistance to other organizations
  • Work with Executive Director to develop organization's programs
  • Conduct and coordinate health education workshops
  • Develop health education workshops with assistance from others
  • Assist the Executive Director with reports and program evaluation

Responsibilities as Clinic Coordinator
  • Recruit and retain volunteer clinic staff
  • Manage and schedule volunteer shifts
  • Conduct the orientation of all new clinic volunteers
  • Act as a liaison between volunteers and staff
  • Conduct inventory and maintain supply of medications, and medical equipment

Qualifications
  • Bilingual in English and Spanish [very strong written and oral] is a must
  • Must be able to work independently
  • Bicultural is strongly preferred
  • Able to use computers, and perform data analysis is strongly preferred
  • A minimum of 2 years experience working in social services, preferably health-related
    services or providing basic needs to people living in poverty is preferred
  • Knowledge of issues affecting day laborers and newly arrived immigrants is preferred
  • Understanding of the CA healthcare system is preferred
  • BA or working towards a higher degree is preferred
  • Can mediate and resolve conflict effectively
  • Can come up with creative solutions to problems
