Street Level Health Project

Case Management

Street Level serves as an entry point for many immigrant community members who are unfamiliar with the local system of health, social services and community resources, as well as a bridge to help them access and navigate these services. Street Level’s case manager and health navigator interns help clients get connected to variety of resources vital for health and wellbeing, including help with getting emergency food and shelter, legal advice, domestic violence support, medical bills, substance use recovery, mental health, specialist healthcare, education/school, and more. Street Level maintains an up-to-date registry of local resources and referrals, to help clients connect with services that fit their specific needs, language, transportation access, income level, and location.
